I really enjoy playing on the server but there are a few things that I dislike about the premium system and map rotations.
First, maps I think that could be removed:
de_aztec - great map for 1v1, 2v2 but not 15v15. Terrorists have very limited opportunity.
de_dust - A classic map, but fairly unbalanced and many people quit during the game. Similar reasons than aztec with player numbers. Could also be removed.
de_dust2_2002_blabla - This is a huge remake of dust 2 with 3 bombspots, sniper towers and new pathways. Nobody seems to like it, so why not remove it?
cs_militia - another unbalanced classic. CTs rarely get anything done on the server.
cs_agency - too small and T sided. again, might be playable with 10 players but not 30.
old inferno, old dust2, de_train_csgo etc. - older versions of actual maps - people get used to the angles on the new map, playing the old maps is very confusing and many dislike it aswell. Also, radar doesn't work.
Maps that I don't really like but I don't know well enough to judge: workout, cruise, rush
EDIT: de_airstrip_csgo (no map, confusing layout) Not sure about this one I kinda like it.
Now to the map vote. It's a little sad, that people just vote to prevent a certain map. I miss "rockthevote" plugin (rtv). It would make us able to change a "bad" map on our own without any admin online and nominate better maps for the upcoming votes.
Also, the maps suggested in the vote are sometimes the maps that we played 1 map before. Unfortunately, it looks like vanilla csgo doesn't provide config options to change it except for the current map not appearing in the vote, but maybe there's a plugin to change that so 4-5 maps have to be played in order for the map to show up again.
Speaking of votes, I think it's impossible to get cheaters votebanned. I know it shouldn't be too easy but 70% instead of 95% sound logical to you?.(could disable voteban if there are less than 6 players)
Next on my list is premium system.
Let me mention first, I'm fine with the tactical nade. Although it can be annoying sometimes, it can be countered and picked up. The only problem is people can get stuck in the grenade -.-"
But free awps... When your team wins the first round and nobody on your team can afford awps, why should the losing team get awps all of a sudden? It's too much of a bad surprise. I think that should be removed. Free Nova, Deagle, Mac10/MP9, CZ Auto / 57 / Tec9, Scout, Galil/Famas or even free defuser / nades are all fine. I don't mind free "eco guns", but awps are too strong to be free.
Also, I think it should be considered to remove premium skins, they are not recognizable as either CT or T if you just see a leg or head. And I have the feeling that they are rendered a little bit later so you see people badly when they peak corners with a custom skin. Especially players new to the server will suffer the most from that.
Lastly, friendly fire /healing system is not a big deal but it shouldn't work with your own grenades (molo). That gives you an advantage of throwing your own molo and running through at enemies which nobody expects, or being able to turn back through your own molo.

EDIT: Maybe we can change sniper restriction by default. I think in a 5 player team maximum should be 2 awps. So we could pick a different plugin that doesn't let us waste money on the awp and then finding out it's restricted, instead restrict the player to buy the gun or refund money / despawn the old gun and set awp+auto restrict on 40% of players.
